A guarantor of a lease completes this rider and signs it in front of a notary public.

What the form asks for
- Tenant's name, apartment and building
- Guarantor's full name, address, driver's license number and state, email, home and cell phone
- Guarantor's signature and date, then the notary's signature
The guarantor form is a rider to the lease. By signing it, the guarantor takes on the same responsibility as the tenant for the rent and the other terms of the lease, and the guarantee stays in effect if the lease is renewed or changed. The signature must be notarized.
