What the form asks for
- The building and apartment applied for
- Current address, how long at it, and the landlord's name and phone number
- Employer, position and annual income, or the school attended
- Name, relationship and age of everyone who will live in the apartment
- Date of birth and Social Security number, and a signature authorizing a credit and employment check
When a lease needs a guarantor, the guarantor fills in the guarantor form and signs it in front of a notary. The office will say which supporting documents to include.
