- Enrollment, cancellation or a change to an existing authorization
- Tenant's name, building and apartment number
- Checking or savings account
- Bank name, routing number and account number
- Account holder's name, address, phone and email
- Tenant's and account holder's signatures
How to return it
- With a blank voided check (checking) or a voided pre-encoded deposit slip (savings) attached
